Most gems will last 2-12months (with the possibility of lasting longer) There are many factors that play a part in Retention (how long they last) but with proper care, they will last you a fair bit of time. 18k gold tooth charms may not last as long as crystal gems due to the weight of the charm being heavier
For a retainer, it depends on the type. If it is a Hawley Retainer (the wire kind), then yes. Just bring it to your appointment and i can make sure the placement of the gems doesn't interfere with it when removing or applying the retainer in the mouth. Unfortunately, if you have the clear plastic retainers (this also applies to Invisalign), it will put too much pressure on your tooth and the gem, so it's not something recommended.
The gems are only 0.4mm or 0.9mm thick and 1.8mm to 3mm in diameter. Initially, something on your tooth will feel different, but a few days after the application, you will get used to the new feeling. If irritation on the lip arises from the gem rubbing against it, you may get it removed by a dentist.
